It was most likely the power supply unit.
"Big Transformer" It can be removed by removing the 4 screws that holds it in the cabinet. Then it slides out.

Update: two days later system started reseting, floated several telephones but had to reconnect because customer had to have them. moved phones around and filled empty ports went from 5 cards to 4. worked ok for 3 or 4 days. still resets but not as often. Resets seem to be random their is not a lot of traffic on system, realestate office with about 29 phones and 2 opx adaptors for voice mail, most of the time there isn't but 4 or 5 people in office at one time.

Is the system resetting at the same time every day?
or is it random.

I would set the system reset time to say 3:00AM and set it to N for disable.

sorry I'm late on this one.

Also the -48v input on the front of the power supply would cause reset's if I am remembering correctly-maybe a bad tri-output supply?

Well I replaced the system with a Vertical SBX... no more reset problems.

The system was properly grounded, it was on a good UPS, There is a Tri Output PS but it feeds two opx adaptors and powers amplified speekers all of which has been floated and the system still reset. There is no patern or time when the system is more likley to reset it was all just random.

By the way I have an Executone IDS 84 phone system with a 2 port VMS for sale...cheap!

Not that it matters now, but you said there was not much traffic on the system. Any chance that 4 hours passed with the system idle during all this?

No, and sometimes it would reset 6 or 7 times in a roll.
