This is issue has been resolved. Thank you all for your input.
Apparently the "new extensions" were on the privacy list of extensions. The following steps were taken to remove them:
1) 501
2) **789#
3) 340#
4) 2#
extension number to be removed
---------------------------------
Other helpful notes:
4) 1 # -> to add extension to privacy list
4) 3 # -> to get a listing of extensions on privacy list
