Disconnect supervision isn't working.
I know you must have it turned on in order to program external call forward, so that's not the problem.
Check the DS timer, listen to the line to be sure your telco is sending a DS signal.

You'll hear a click and a buzz as the line is opened and loop current drops to 0 momentarily.
