I have a user in Rochester, NY, who has in issue with his 8662 phone on an Inter-Tel 5000. All red LED lights on the phone flash erratically, and he gets the following messages: "Download in Progress", then "ERROR - TFTP Fail".
How do we fix this?

Hold down the 7 & 8 keys while the fone is booting. You should get a menu, release the keys. Select Programming Menu. Navigate right until you see 'TFTP Server URL'. Select 'Revert'. Navigate right until you can save changes. Save and reboot.

It sounds like the phone is hitting a valid TFTP server but the config file it's parsing is not structured properly. Do the 7+8 combo and check the TFTP IP. Is it valid? Does it run a TFTP daemon? If there a valid globalcfg.cfg or [mac]cfg.cfg file?
