Update:
Issue appears to be with the interface between the DMS 100 Central Office Switch and the OS 7100. We're in the old BellSouth footprint so we see a lot of DMS connections.
We are able to ask AT&T to "bounce" the circuit or force the D Channel and it will pop right up. Otherwise takes around 1 and 1/2 hours to come up on its own. Once it's up it works properly until you need to power down or reset the system.
For those that asked there is an Adtran CSU between the smart jack and the OS 7100.

Is it an Adtran that supports B8ZS ESF or just one that support D4/SF Framing? It needs to be the ESF version.

Yes the Adtran supports the B8ZS/ESF.
May not have mentioned earlier that I was able to take TEPRIa and move it into a OS 7200 keeping the AT&T circuit and Adtran CSU constant and that it would pop right up working properly.
Issue has been isolated to an issue with the OS 7100 and the PRI coming from a DMS 100 Central Office.

I have come an issue with the carrier giving DMS 100 but on the system changing it to NI2 and it worked. Why ??? but something to try
