Obviously this is an old telcom junction box of sorts, but what is it officially called? It has been a pain -- would AT&T replace it with something more modern (and weather resistant)?

What it is should be on the cover. It's a protected terminal that someone has removed all the carbon protectors from. They now use a building terminal for those and all you can do is call to see if they would replace it. As it stands there is no pair protection.

Here's some of the different protectors. Around page 9 or so is a 116C terminal protector, which is a smaller version of what you're showing. Poke here

It seems to only be missing the carbons from pairs 14 and 16.

Some of the later carbons had the Hex head that a can wrench fit

Even if all the carbons were missing, it's not a big deal IF it's been converted to just a junction with a newer protection device farther on, which I see from time to time around here.

To my eyes the top of terminal has a lead sleeve feeding the unit.

The hex head carbons i think had a gas hex module, which converted to Gas rather carbon protection.
Thus if you were have trouble may also indicate bad lead cable.
