Larry, I can't shake the nagging feeling that I also did something else but just don't find anything in my notes. The only suggestions that come to mind are to temporarily change other items like disabling DISA CO-TO-CO and changing SLT Flash and Bounce timers.
If anything else comes to mind, I will follow it up. Disconnect supervision must have played a part though because when I arrived, none of the ports had released and enough time should have elapsed for the timers to have kicked in. I think I had to compensate for the failure of the provider to always properly disconnect. The problem may have surfaced once again after all of my efforts but cleared itself before I arrived.

Thanks Dave. DISA CO-TO-CO is already disabled, and I was considering changing the SLT Flash Timer. Not sure if I should try shortening the timer or extending it.

There is the SLT hook flash timer, SLT hook bounce timer and the PBX/Centrex flash timer. I don't think adjusting the last one is going to make a difference in your case. The first two might.
The slt hook flash timer has to be set longer than the fixed timer of a flash button on a phone for a transfer to work.
The hook flash bounce timer if set long, any short flashes are ignored. This is the timer I would try first.
