I have a customer with a 11 year old legend. T1, PRI. The operator is saying that the phone puts callers on hold in the middle of the conversation and she needs to press the flashing button to resume the call. Think it's the phone or the port/system?? Qwest said the T1 is ok.

I am rather sure it's the phone, or the cord to the phone.
What you are describing is exactly what happens when you unplug a phone during a conversation, and then plug it back in.
So, now you have some things you can look at.

Well it wasn't the phone. Installed a "factory" refurb, same thing happening. Just ordered another 4x8 card, hope this fixes it!

Still having problems even with the new card. The system has 3 power supplies. Should I try swaping the power supply with one of the expansion cabinets?

Though it doesn't appear to be the phone or mounting cord, it could just as easily be a "bisquit" jack under the desk that is being inadvertently kicked about.
If this happens frequently, it may make sense to plug the phone directly into the 408MLX port and eliminate the board as the fault source. Even so, I still like your approach as those 408 modules are pretty cheap!
I had a user complain about a similar problem not long ago, and it DID turn out to be a faulty port. However, his phone tended to die altogether and not return to service unless he plugged/unplugged his phone.

That was the first thing I did, move the phone to a new location. Still thinking about changing the power supply. I did unplug the DSS consoles (2) and asked them to try it for a day. We Will see.
