Has anyone used a video handset like the Polycom 1500D with an Avaya IP office system?

There are a couple of devices supported by Avaya with embedded video.
I beleive that the VVX1500 is one of them. If you are an avaya partner, the knowledgebase will have documentation on setting it up.
