This is how I set up every IPO that I do: https://www.ipofficehelp.com/HOWTO_BypassDial9.aspxIt is designed to allow you to call numbers back when they appear in your call log, because the incoming caller id will be 10 digits long, beginning with the area code, even if local. You can also add numbers in the log to your contacts, without having to edit the number to include the 9 and the 1. Although not the original intent, a consequence is that you can now dial outgoing calls with 10 digits, no leading 1 for long distance, and all 10 digits for local calls. It does not interfere with the extension numbers.

I like Kyles setup, but if you make them dial 1 for every call, then you create 1 short code. Because you have no 1XX extensions, there wont be a problem.
