Sounds like a can of worms. I would rip it out and replace with a DSX 40 or 824 pan, be quick and easy if wire is star config. If looped I would have never started. Heck just bring a KSU to site, plug in lines and a couple phones and test.

Update:
Original loop curent value: 50mA = bad crosstalk. Installed Mike Sandman loop current regulators. 25mA guaranteed, regardles of what is coming from telco. Measured value of 24.9mA
Acquired and installed adjustable loop current regulators. No complaint since. Go figure: 24.9 mA of loop current lead to calls constantly dropped on transfer or even on hold from afar.
A measured 26.6 mA did the trick.
